• • These are Student Physical and Health Information Forms that need to be filled out and on file at the school when a child is entering Kindergarten or 7th grade.
  • Also, if the student will be participating in sports the forms need to be filled out and given to the school.
  • Please print off the front and back and take with you to your physical appointment.
  • There is also a School Vision Evaluation Report Form that all Kindergarteners need to have filled out before entering Kindergarten.
